A number that opens a door

Most health calculators hand you a number that means something about you. This one hands you a number that means something to a system.

Pack-years exist because doctors needed a way to compare smoking histories that were not remotely alike. Someone who smoked forty a day for ten years and someone who smoked ten a day for forty years have had very different lives and very similar cumulative exposure, and medicine needed one figure that captured both. So it built one, out of the two things that matter: how much, and for how long.

The reason to know yours is practical rather than philosophical. One specific pack-year figure, 20, is a threshold in lung cancer screening guidelines. Cross it, alongside a couple of other criteria, and you may be eligible for an annual scan that catches cancer early enough to matter. That is the whole point of this page: not to grade your past, but to tell you whether a door is open.

What a pack-year actually is

One pack-year is smoking an average of 20 cigarettes, one standard pack, every day for one year.

The sum is simply how many packs a day you smoked, multiplied by how many years you smoked them:

Pack-years = (cigarettes per day ÷ 20) × years smoked

The division by 20 turns your daily cigarettes into a fraction of a pack. Ten a day is half a pack, so 0.5. Thirty a day is 1.5 packs. Then multiply by the years.

Two things about the metric are worth noticing. It counts only the years you were actually smoking, not your age and not the years since you stopped. And it treats intensity and duration as interchangeable, which is a genuine simplification and one that researchers have argued about, but it has been the standard shorthand since the lung cancer studies of the 1950s and 60s established the dose-response relationship it encodes.

A worked example

Someone who smoked 20 cigarettes a day for 15 years:

Packs per day = 20 ÷ 20 = 1
Pack-years = 1 × 15 = 15 pack-years

Which puts them below the 20 pack-year screening threshold, though not by much, and the threshold is not the only thing that matters.

Try it with ten a day for the same fifteen years and you get 7.5 pack-years. Forty a day for those fifteen years gives 30. The arithmetic is not complicated, which is exactly why it caught on.

Same number, many routes

This is the part that makes pack-years click. All of these are 20 pack-years:

The last row is there to show where the model strains. Eighty years of smoking five a day is not really available to anyone, and treating it as identical to ten years of heavy smoking is a stretch. Duration and intensity are not perfectly interchangeable in reality, and the evidence suggests duration may matter more than the formula implies.

But the middle rows are the useful insight, and they surprise people. A light smoker of long standing can quietly accumulate the same pack-year total as someone who thinks of themselves as a much heavier smoker. "Only ten a day" for forty years is 20 pack-years, and that is over the screening line.

The pack size box, and why 20 is usually the right answer

The tool lets you change the pack size, which is a thoughtful touch, since packs are not 20 everywhere. Australia sells 25s. Plenty of places sell 10s, or 30s, or whatever the tax regime encourages.

But there is a catch worth understanding before you change it. The clinical definition of a pack-year is anchored to 20 cigarettes, full stop, regardless of what your local shop sells. It is a fixed unit, like a nautical mile. So if you smoke 20 a day and set the pack size to 25 because that is what you buy, the tool will tell you that fifteen years of smoking is 12 pack-years rather than 15. It is not wrong about the arithmetic you asked for. It is just no longer the number your doctor means.

So the guidance is straightforward: leave the pack size at 20 if you want a figure that means anything to a clinician, and enter your real cigarettes per day. Use the box only if you know why you are changing it. If you buy 25s and smoke a pack a day, that is 25 cigarettes a day, not one pack a day, and 25 in the cigarettes box with 20 in the pack size box gives the right answer.

The 20 pack-year threshold and what it unlocks

Here is the reason this calculator is worth running.

The US Preventive Services Task Force recommends annual lung cancer screening with a low-dose CT scan for people who meet all three of these:

  • Aged 50 to 80.
  • A smoking history of 20 pack-years or more.
  • Currently smoking, or quit within the past 15 years.

Those criteria were widened in 2021. The age range used to start at 55 and the threshold used to be 30 pack-years, so a substantial group of people who were previously ineligible now qualify, and many of them do not know it. If you stopped smoking twelve years ago and have never thought about screening, that third criterion may still include you.

Why it matters: low-dose CT screening in high-risk groups has been shown to cut lung cancer deaths by roughly a fifth, and it works by finding disease early, while it is still treatable. Lung cancer is unusually good at staying silent until it is advanced, which is precisely why a scan beats waiting for symptoms.

Two caveats worth stating plainly. These are US criteria, and other countries run different programmes with different thresholds, so check what applies where you live. And this calculator does not enrol you in anything. It gives you a number to take to a doctor, who will weigh it against the rest of your picture. If you are near or over 20 pack-years and in that age band, that conversation is worth booking.

If you smoked different amounts at different times

Almost nobody smoked the same amount for their whole smoking life, and the tool takes one figure for cigarettes per day, so here is how to handle a real history.

Split it into periods and add them up. Someone who smoked 10 a day through their twenties, then 20 a day for fifteen years, then 30 a day for five:

(10 ÷ 20) × 10 years = 5 pack-years
(20 ÷ 20) × 15 years = 15 pack-years
(30 ÷ 20) × 5 years = 7.5 pack-years
Total = 27.5 pack-years

Run each period through the calculator separately and add the answers. Averaging your cigarettes per day across the whole span gets you close, but doing it in blocks is more honest and takes a minute.

And do not count gaps. If you quit for three years and started again, those three years are not smoking years. Count only the years you actually smoked.

Quitting does not lower your pack-years, and that is not the point

This one lands badly with people, so it is worth explaining properly.

Your pack-year total is a record of exposure that already happened. It cannot go down, because the past does not change. Quit today and your pack-years stop climbing, permanently, but they do not reverse. That is why the screening criteria treat pack-years and quit date as two separate questions: one asks what you accumulated, the other asks how long ago you stopped.

What quitting does change is everything forward-looking. Risk of lung cancer, heart disease and stroke all fall after stopping, and they keep falling for years. The risk does not return to that of someone who never smoked, and pretending otherwise would be dishonest, but the trajectory bends and it bends a lot. A frozen pack-year number alongside a falling risk curve is a good outcome, not a contradiction.

So a high number here is not a verdict and it is not a reason to conclude the damage is done. It is a measurement of the road behind you, which is useful mainly because of what it tells you to do about the road ahead. If you want help stopping, your doctor or pharmacist can talk you through options, and the evidence is clear that support and medication together work considerably better than willpower alone.

The other two numbers it shows you

Alongside your pack-years, the tool reports how many cigarettes and how many packs you have smoked in total. For our 20-a-day, 15-year example, that comes to about 109,572 cigarettes and roughly 5,479 packs.

These are not clinical figures. No doctor asks for them and no guideline uses them. They are the same history multiplied out into a count, and they exist because a raw total lands differently than a tidy metric does.

Take that for whatever it is worth to you. Some people find a number like that clarifying; others find it just a number. Both are reasonable reactions, and the pack-year figure above is the one that actually does anything.

What the number cannot capture

Pack-years are a decent shorthand and a crude instrument. What they miss:

  • How you smoked. Depth of inhalation, how much of each cigarette you smoked, whether you re-lit. Two people at 20 pack-years can have taken in quite different amounts of smoke.
  • What you smoked. Tar and nicotine yields have changed enormously over the decades, and a cigarette in 1975 is not a cigarette in 2025.
  • Roll-ups, pipes, cigars and shisha. The metric is built on manufactured cigarettes. Roll-ups in particular do not convert cleanly, and people routinely underestimate them. If that is your history, describe it to your doctor rather than forcing it into this box.
  • Vaping. Not included at all. Pack-years predate it by half a century and there is no accepted conversion.
  • Second-hand smoke. A real exposure with no place in this formula.
  • When you started. Beginning at 14 rather than 24 appears to matter beyond the extra years it adds, and the arithmetic cannot see that.
  • Everything else about you. Family history, occupational exposures like asbestos or radon, and existing lung disease all move your actual risk, and none of them appear here.

All of which is the honest summary: this is one input into a conversation, not a risk score. It happens to be the input that guidelines are written around, which is what makes it worth calculating.

Questions people ask

What is 20 cigarettes a day for 15 years in pack-years?

15 pack-years. Twenty a day is exactly one pack, so the sum is 1 × 15. That sits below the 20 pack-year screening threshold, though if you carry on it will cross it in five more years, and the threshold is only one part of the picture your doctor would consider.

Do my pack-years go down now that I have quit?

No. Pack-years record exposure that already happened, so the total is fixed the day you stop and never falls. What falls is your forward risk, which drops steadily for years afterwards. The screening criteria account for this by asking separately how long ago you quit, with 15 years being the cut-off.

Am I eligible for a lung cancer screening scan?

Under US guidance you may be, if you are 50 to 80, have 20 or more pack-years, and either still smoke or quit within the last 15 years. All three have to be true. This page cannot enrol you in anything, and other countries use different criteria, so take the number to a doctor and ask.

I buy packs of 25. What should I put in the pack size box?

Leave it at 20 and enter your real cigarettes per day. The clinical pack-year is defined against a 20-cigarette pack everywhere in the world, whatever your shop sells. If you smoke one 25-pack a day, that is 25 in the cigarettes box and 20 in the pack size box, giving 1.25 packs a day.

How do I count roll-ups?

Awkwardly, and approximately. There is no standard conversion, and estimates of how much tobacco goes into a hand-rolled cigarette vary a lot. Most people count each roll-up as one cigarette, which probably understates it. If your history is mainly roll-ups, tell your doctor that rather than relying on a number from here.
